Pricing:

• $65 one-time sign-up fee

• $95 per month

Includes:

• Everything in Virtual Essentials

• Initial in-home visit for new members, follow up telemedicine

• Second Home Visit Free (used within 12 months of first home visit)

• Follow-up telemedicine visits

• Expanded lab and diagnostic ordering

• Care coordination, including post-hospital discharge follow-up, specialist communication, and care plan alignment

• Additional Home Visits: $40 per visit (within service area; subject to availability)

Designed for: Adults with chronic conditions or increased need for access and coordination.

B2 Complex Care Membership is designed for patients with multiple chronic conditions, recent hospitalizations, or advanced care needs. Availability is limited to ensure quality and access.

Pricing:

• $80 one-time sign-up fee

• $135-195 per month (based on care intensity and visit frequency)

Includes:

• Everything in B2 Virtual + First Home Visit

• Ongoing in-home visits (typically quarterly or more frequently as indicated and based on availability)

• Expanded care coordination

• Hospital, specialist, and caregiver communication

• Additional Home Visits: $15 per visit (within service area; subject to availability)

Designed For: Patients with multiple chronic conditions, recent hospitalizations, or advanced care needs.

B2 Direct Care is currently offering introductory pricing for new members during its initial launch period.

Members who enroll during this phase will have their registration fee and monthly membership rate locked in for 12 months from their start date.

As the practice grows and services expand, pricing is expected to increase for future members. Current members will always receive advance notice prior to any changes after their initial 12-month period.
