Membership Pricing
$75 / month
$55 one-time enrollment fee
Membership fees support a direct, ongoing relationship with your physician and are not billed to insurance.
There are no contracts. Membership is billed monthly.
What your membership includes
A comprehensive initial telemedicine visit (60 minutes)
Ongoing telemedicine visits, scheduled or as needed
Direct secure access via messaging, phone, or email
Medication management and prescription coordination
Care coordination with specialists and other providers
Guidance around labs, diagnostics, referrals, and follow-up care
Continuity during health transitions, including hospital discharge when applicable

What’s not included in the monthly membership
Some services fall outside the scope of the monthly membership and are coordinated separately.

Home Visits
$75 per visit (60 minutes)

During the initial launch period, new members may be eligible, when appropriate, for a reduced-rate first home visit.
Hospital-to-Home Navigation
$250 (30 days) | $400 (60 days)
Short-term support following hospital discharge, including medication reconciliation, care coordination, and early follow-up.
If a patient later enrolls in the Direct Care Membership, the enrollment fee is waived.
Comprehensive Chart Review & Consultation
$250–$500 (one time)
A consultative service for patients seeking a structured review of medical records and recommendations. This service does not establish an ongoing primary care relationship.
