The Direct Care Membership establishes a longitudinal, physician–patient relationship focused on accessible, telemedicine-based primary care and thoughtful medical guidance.

Many members are healthy most of the time. They value having a physician available for quick questions, early guidance, and decision-making that helps avoid unnecessary urgent care or emergency visits — especially in high-deductible or self-pay situations.

Care is provided within the scope of Dr. Bost’s training and clinical judgment.

What’s Included in the Monthly Membership

    • Comprehensive initial telemedicine visit

    • Follow-up telemedicine visits, scheduled or as needed

    • Evaluation and management of acute and chronic conditions

    • Preventive care and health maintenance guidance

    • Secure video visits

    • Secure messaging via text or email to reach Dr. Bost directly

    • Care delivered according to established telemedicine standards

    • Review and interpretation of outside records to create a clear, centralized medical record

    • Coordination with specialists and other providers when appropriate

    • Guidance around diagnostic testing, referrals, and follow-up care

    • Support navigating the healthcare system as it relates to your care

    • If hospitalized, Dr. Bost may “follow along” remotely and schedule a post-discharge visit within 48 hours when feasible

    • Prescription management and refills

    • Medication reconciliation across providers

    • Guidance on medication use, risks, and alternatives

    Note: Chronic opioid or controlled-substance pain management is not provided

    • Ordering of labs and diagnostic studies when clinically indicated

    • Interpretation and discussion of results

    • Access to reduced cash pricing for certain labs and diagnostics (third-party fees apply)

    • Optional in-home phlebotomy services may be available (additional fees apply)

Services Not Included in the Monthly Membership Fee

The following services are not included in the monthly membership fee and may require additional fees or separate agreements:

  • Home visits (available for active members when clinically appropriate; charged per visit)

  • Hospital care or inpatient services

  • Emergency department care

  • Procedures or services outside the scope of primary care

  • Services provided by third parties (e.g., laboratories, imaging centers, specialists)

  • Non-membership programs or consultative services offered under separate agreements
