B2 Direct Care Membership

$55 one-time enrollment fee

$75 / month

·      This membership establishes a longitudinal, physician–patient relationship centered on telemedicine-based primary care, with option for home visits.

Includes:

·       Initial 60-minute, new member telemedicine visit

·       Access to calling/texting /email for convenient access to your physician

·       Scheduled and on demand tele-visits

·       Medication management and prescription coordination

·       Laboratory tests and diagnostic testing at reduced cash-based membership rates

·       Ongoing care coordination and longitudinal medical guidance including routine primary care of coordinating care with specialists, including coordination if hospitalization occurs

 

The B2 Direct Care Membership is designed to provide access, continuity, and clarity through a direct relationship with a physician who knows your medical history and helps guide care over time.

Home Visits

Standard price: $75 per visit

·       60 minutes

·       Available to active members only

·       Offered within the defined service area (see below)

·       Subject to clinical judgment and physician availability

Home visits are optional, episodic services and are not included in the monthly membership fee. Any promotional pricing or discount  does not alter their status as non-included services.

 

 

New Member Home Visit Incentive (Initial Launch Only)

To encourage early engagement and relationship-building, new members may be eligible for a reduced-rate initial home visit.

  • 50% off the first home visit

  • Available within the first 90 days of membership

  • Offered when clinically appropriate and based on availability

 

This incentive is time-limited, applies to the initial launch cohort only, and may be modified or discontinued in the future.

 

Home Visit Service Area

 

Home visits are generally available to members located within 10 miles of the Colorado State Capitol.

Requests outside this service area may be declined or, when appropriate, offered with a mileage surcharge at the physician’s discretion.

 

 Family Membership Pricing

Additional family member policy (applies to all tiers):

  1. Same sign-up fee as the primary member minus $10

  2. Same monthly fee as the primary member minus $10

 

INTRODUCTORY PRICING

B2 Direct Care is currently offering introductory pricing during its initial launch period.

  • Registration fees and monthly membership rates are locked for twelve (12) months from the enrollment date

  • Pricing may increase for future members as the practice grows and services expand

  • Current members will always receive advance notice prior to any changes after their initial 12-month period

Our goal is to grow thoughtfully—without sacrificing access, quality, or continuity of care.

 

NON-MEMBERSHIP PROGRAMS & SERVICES

(Separate agreements will apply)

The following services are available to members and non-members and are not part of the Direct Care Membership.


Hospital-to-Home Navigation Package

$250 – 30 days

$400 – 60 days

A short-term program designed to support patients during the vulnerable transition following hospital discharge.

Includes:

·       Review of discharge summaries and care plans from all care teams while hospitalized (internal medicine/pediatrics, surgeons, specialists such as cardiologists, nephrologists etc)

·       Medication reconciliation

·       Early post-discharge follow-up

·       Short-term care navigation and coordination

·       Participation in discharge planning when available

·       Home visit when appropriate and within the defined service area (subject to availability)

If a patient enrolls in the Direct Care Membership after completing this program, the membership registration fee is waived as is the first month of membership.

 

Comprehensive Chart Review & Medical Consultation

$250–$500 (one-time, based on complexity)

Includes:

·       Comprehensive review of medical records

·       Identification of key diagnoses, medications, and gaps in care

·       Dedicated consultation to review findings and recommendations

This service is consultative in nature and does not establish an ongoing primary care relationship.

If a patient enrolls in the Direct Care Membership after completing this program, the membership registration fee is waived.
