You've just been discharged from the hospital. You have a bag of new medications, a stack of paperwork, and a follow-up appointment that's two weeks away.

What Is Hospital-to-Home?
B2 Hospital-to-Home is a 30-day transitional care program that starts within 24 hours of your discharge. You're paired with Dr. Brian Bost — a board-certified physician in both adult and pediatric medicine, and a practicing hospitalist who understands exactly what happened during your hospital stay.
This isn't a phone tree. It's not a follow-up survey. It's a real doctor, reviewing every one of your medications on camera, making sure your discharge instructions make sense, and staying with you for the full 30 days until you're safely back on your feet.
How It Works
Day-after-discharge visit
Within 24 hours of leaving the hospital, you'll have a video visit with Dr. Bost. Together, you'll go through every medication — bottle by bottle, on camera — and review your discharge plan line by line.
30 days of support
For the next month, we check in regularly. We coordinate with your primary care doctor and your specialists. We help with prescriptions, referrals, and any questions that come up.
Safe handoff
At the end of 30 days, your primary care doctor gets a complete summary — reconciled medication list, what we found, what we did, and what to watch for next.
Your Primary Care Doctor Is Still Your Doctor
This is important: Hospital-to-Home does not replace your existing doctor. If you have a primary care provider you already see, that relationship stays exactly the same. We're here to fill the gap between the hospital and your next appointment with them.
During your 30 days with us, we actively coordinate with your PCP — sharing medication updates, flagging concerns, and making sure your follow-up appointment happens.
If you don't have a primary care doctor right now, we'll help you find one.
And if you decide you'd like to stay on with Dr. Bost as your doctor after the 30 days? You can. But there's no pressure, no obligation, and no hard sell. That's your call.

Dr Brian is board-certified in both Internal Medicine and Pediatrics. He's a practicing hospitalist who understands the complexities of hospital care — and what it takes to recover safely at home.
